Westlake High School 1:1 MacBook Program Parent/Guardian Acknowledgment of Responsibility

I acknowledge that my child has been issued a district-owned MacBook computer and charger for educational use. I understand and agree to the following:

1. Ownership and Normal Use

The device remains the property of Mount Pleasant CSD at all times. The District does not charge families for issues covered under the manufacturer's warranty, including manufacturer defects and certain normal-usage repairs.

2. Parent/Guardian Financial Responsibility

The District has secured comprehensive warranty coverage for each device. However, if my child's device requires repair for reasons not covered under the warranty, I understand I am responsible for the cost of repair, up to the cost of a replacement device. This includes, but is not limited to, cracked screens, spill/liquid damage, and physical damage from drops or mishandling.

I understand this responsibility does not apply to normal wear and tear or manufacturer defects, which remain the District's responsibility under warranty.

The cost of repair or replacement billed to the family will not exceed $494, the current replacement cost of the device.

3. Lost Devices

If the device is lost, I am responsible for the replacement cost of the device, not to exceed $494.

4. Theft

If the device is stolen, the family must:

  • Report the theft to the Technology Department immediately, and no later than [X school days] of discovery
  • File a police report and provide a copy to the District
  • Understand that the District's insurance claim must be filed within 30 days of the theft; failure to meet this window will result in the family being responsible for the full replacement cost, not to exceed $494
  • Pay a $250 deductible if the theft claim is approved and filed within the required timeframe

Theft not reported promptly, or not accompanied by a police report, will be treated as a lost device and billed at the replacement cost (not to exceed $494) rather than the theft deductible.

5. Intentional Damage

Intentional damage or vandalism of district-owned equipment will be referred to building administration for possible disciplinary action in accordance with the Code of Conduct.

6. Reporting Requirement

All damage, malfunction, or loss must be immediately reported to the Technology Department. 

7. Payment

Fees will be billed through the Mount Pleasant Central School District Business Office.

Students with an outstanding balance on their account will be issued a Chromebook in place of a MacBook until the account has been resolved.

I have read and understand this agreement and agree to be financially responsible as described above.
