Parent/Guardian Acknowledgment of Responsibility
I acknowledge that my child has been issued a district-owned Chromebook, charger, and case for educational use. I understand and agree to the following:
1. Ownership and Normal Use
The device remains the property of Mount Pleasant CSD at all times. The District does not charge families for minor issues arising from normal use or manufacturer.
2. Parent/Guardian Financial Responsibility
If my child's device is accidentally damaged and I have not purchased optional device insurance, I understand I am responsible for the cost of repair, up to the cost of a replacement device ($200 cap). This includes, but is not limited to, cracked screens, spill/liquid damage, and physical damage from drops or mishandling.
I understand this responsibility does not apply to normal wear and tear or manufacturer defects, which remain the District's responsibility regardless of insurance status.
3. Lost Devices
If the device is lost, I am responsible for up to a $200 replacement fee, reflecting the current replacement value of the device. This fee applies whether or not optional insurance has been purchased, as the district's device insurance does not cover unexplained loss.
4. Optional Device Insurance
The District offers optional device protection through Worth Ave Group for the 2026–2027 school year:

This plan covers accidental damage, cracked screens, spills/liquid submersion, fire/flood/natural disaster, power surge, and vandalism (and theft, if that option is selected). It does not cover manufacturer defects/mechanical failure, cosmetic damage, normal wear and tear, or unexplained loss.
Because the District's Chromebooks are being phased out in favor of MacBooks over the next several years, claims are generally resolved through repair or replacement with a comparable used device rather than a new unit.

If insurance is purchased and a device is damaged in a covered incident, a claim will be filed with Worth Ave Group rather than paying the District out of pocket. If theft coverage was purchased and the device is stolen, a police report will be required to file the claim (see theft procedure below).
5. Theft
If the device is stolen:
• Report the theft to the Technology Department immediately
• File a police report and provide a copy to the District
• If theft coverage was purchased, a claim will be submitted to Worth Ave Group per their claims process
• If theft coverage was not purchased, or a police report is not provided, the incident will be treated as a lost device and billed at the replacement fee, up to $200.
6. Reporting Requirement
All damage, malfunction, or loss must be reported to the Technology Department as soon as possible. Delayed reporting may affect eligibility for insurance claims or increase family financial responsibility.
7. Payment
Fees will be billed through the Mount Pleasant Central School District Business Office.
8. Intentional Damage
Intentional damage or vandalism of district-owned equipment will be to building administration for possible disciplinary action in accordance with the Code of Conduct.
